California Children's Services (CCS) Program

The California Children's Services Program coordinates and authorizes specialized medical care and provides rehabilitation for children with special health care needs, from birth to 21 years of age, who have a CCS eligible medical condition, that are physically disabling, or that need to be treated with medicines, surgery, or rehabilitation.

Client receives:

  • Diagnostic evaluation;
  • Treatment services;
  • Occupational and physical therapy; and
  • Medical case management

Client eligibility:

  • Children (birth to 21 years of age) who have a CCS eligible medical condition
  • Resident of Del Norte County
  • Meets CCS financial eligibility*
  • Earn less than $40,000 (adjusted gross income), OR
  • Child's medical costs are expected to exceed 20°/o of adjusted gross income, OR
  • Child has state full scope Medi-Cal without share of cost, OR
  • Child has Partnership Medi-Cal without a share of cost, OR
  • Child is covered under the Optional Targeted Low Income Children Program (OTLICP)
*There is no financial eligibility requirement for diagnostic services, the Medical Therapy Program, and High Risk Infant Follow Up.

CCS Paneled Providers receive:

  • Technical assistance and consultation
  • Information on client eligibility status

Whole Child Model

Per SB586, Del Norte County is a Whole Child Model (WCM) County where Partnership HealthPlan of California provides case management services and authorizations for CCS children and youth who are also Partnership Medi-Cal beneficiaries.  Del Norte CCS processes referrals and determines financial and residential eligibility.  The State’s Integrated Systems of Care Division determines medical eligibility.  


Partnership’s WCM Family Advisory Committee (FAC)

PHC’s WCM Family Advisory Committee is a forum where you, as your child’s advocate, can share information, give input to PHC, and advocate and network with other FAC members with children in the CCS Program. The goal of the FAC is to improve the quality of care and services of the WCM program.

The FAC meetings are held virtually every quarter. The upcoming FAC meeting dates are:

  • February 27, 2024
  • May 28, 2024
  • August 27, 2024
  • November 19, 2024
