The purpose of Child Health and Disability Prevention (CHDP) Program is to provide for the early detection and prevention of disease in children. If a condition is discovered, referral and follow-up assistance is provided. The local administration office offers outreach, education, basic information and referral, provider relations and quality assurance/care coordination for medical providers.
Client Services:
This federal and state-funded program provides well-child exams that include physical exam, dental and nutrition evaluation, hearing and vision screening, testing for anemia, diabetes, TB and lead poisoning, and all immunizations. These exams can also be used for camp, sports and school entry.
The CHDP Program is also a "Gateway" for uninsured families to gain access to Medi-Cal services through a pre-enrollment process.

Please see our List of CHDP Provider Offices that provide CHDP exams, including camp/sports physicals and school entry or pre-school. Some of these offices may not accept your Medi-Cal card for other services. When you call for an appointment, be sure to tell the office you are requesting a CHDP exam, a pre-school exam, or a camp/sports physical for your child.
